What is a Voiding Diary?
A voiding diary is a daily record of your bladder activity and urination behaviour. The diary usually contains objective data on your voiding patterns and output, and any incidents of incontinence and its possible causes.
Instructions for Completing a 24-hour Period Voiding Diary
- Please complete the voiding diary for the entire 24-hour period.
- Print your name and date at the top of the diary on the day you start.
How do you Record in the diary?
- Time of Day: Please use one voiding diary sheet per 24-hour period. Circle your wake-up time and bedtime in the “Time" column. Start recording all fluid intake and urinary events in the appropriate time slots throughout the 24-hour period.
- Fluid Intake: In the column marked “Fluid Intake” record the total amount of fluids you drank (in oz, ml or cc) during the 24-hour period.
- Toilet Voids: Make sure the urinary hat is placed over the toilet each time you urinate. Record the amount of urine you void (in oz, ml or cc) at each occasion over the 24-hour period.
- Amount of Urine Drained via Catheter: In the column titled “Amount of Urine Drained via Catheter”, record the amount of urine (in oz, ml or cc) that you collected or drained. Notate whether this was a residual or catheter void; for example, 4 oz residual [R], or 6 oz catheter [C]. Please leave this column blank if you did not use a catheter.
- Leaks: Record a tick for each time you had involuntary urine loss (even a small amount) before you made it to the toilet.
- Pad Changes: Mark this column for each time you changed your pad. If the pad was dry, indicate “D”. If the pad was wet with urine, describe the amount as small, moderate or large.
